چکیده

A 71-year-old man complained of 5 years of progressive postural instability, more intense when standing up from bed, especially in darkness and during the day when walking on street. It lasted a few seconds and resolved spontaneously. No reference to vegetative symptoms, hearing loss or rotatory movements. Relevant personal clinical antecedents were slowly progressive visual loss that began at age 20, followed by the diagnosis of pigmentary glaucoma at age 40, and three episodes of retinal detachment repair (twice in the right eye and once on left eye, during the last two decades), and 6 months ago he underwent a combined surgery directed to right glaucoma and cataract. Other antecedents were lymph node tuberculosis and radical prostatectomy three years ago and benign paroxysmal positional vertigo of horizontal and posterior right semicircular canals treated with repositioning maneuvers two years ago.

Acquired Periodic Alternating Nystagmus

대한신경과학회지 22권 5호 548 Periodic alternating nystagmus (PAN) is a rare type of spontaneous horizontal nystagmus, characterized by periodical reverse of direction with a transition period in between. PAN can occur in both congenital and acquired forms. We report a patient with acquired PAN associated with meningoencephalitis. To our knowledge, only a case of congenital PAN has been previously report...

Anti-GAD antibodies and periodic alternating nystagmus.

BACKGROUND Autoantibodies directed against glutamic acid decarboxylase (GAD-Ab) have recently been described in a few patients with progressive cerebellar ataxia, suggesting an autoimmune physiopathologic mechanism. OBJECTIVE To determine the exact role of GAD-Ab and gamma-aminobutyric acid (GABA)-ergic neurotransmission in the pathogenesis of cerebellar ataxia. DESIGN Case report. SETTIN...

Analysis of a case of periodic alternating nystagmus.

A patient is described in whom there was periodic deviation of the eyes in darkness associated with alternating nystagmus. An exactly similar situation occurs in normal man subjected to a pendular stimulus. It is proposed that in this patient there was alternating activity of the vestibular nuclei on either side of the brain-stem as a result of a removal of cerebellar inhibition, and that this ...

The clinical and molecular genetic features of idiopathic infantile periodic alternating nystagmus.

Periodic alternating nystagmus consists of involuntary oscillations of the eyes with cyclical changes of nystagmus direction. It can occur during infancy (e.g. idiopathic infantile periodic alternating nystagmus) or later in life. Acquired forms are often associated with cerebellar dysfunction arising due to instability of the optokinetic-vestibular systems. Idiopathic infantile periodic altern...

Teaching Video NeuroImages: periodic alternating nystagmus evident only in darkness.

An 81-year-old man with retinitis pigmentosa had severe, slowly progressive visual loss that began at age 20. Intermittent nystagmus was observed for 1 decade. Examination revealed severe retinal atrophy and periodic alternating nystagmus (PAN) that occurred only in darkness (video on the Neurology® Web site at www.neurology.org). PAN refers to horizontal nystagmus that periodically reverses di...

Two types of isolated epileptic nystagmus: case report.

Epileptic nystagmus (EN) is a quick, repetitive jerky movement of the eyeball caused by seizure activity, unaccompanied by other ictal phenomena rare. Here, we described two cases, one characterized by binocular and the other by monocular isolated epileptic nystagmus (IEN), and we identified the characteristics of the etiology, clinical manifestations, electroencephalogram, imaging, treatment a...
